Criteria

Standard Dosing

Low Dose	400 mg in 40 ml over 15 min using 0.22 micron filter	480 mg in 48 ml over 2 hrs using 0.22 micron filter
High Dose	800 mg in 80 ml over 30 min using 0.22 micron filter	960 mg in 96 ml over 2 hrs using 0.22 micron filter

Reconstitute to 10 mg/ml with sterile water

Choose Factor XA Inhibitor and Dose

Factor : Please choose a type Apixaban (Eliquis) Rivaroxaban (Xarelto)

Dosing: Please choose a Factor XA first

Choose When Last Dose of Factor XA was taken

Time taken: Please choose last time taken Less than 8 hours or Unknown Greater or Equal to 8 hours

Prepare Dose

Reconstitution

Reconstituted vials are stable 8 hours at room temp, 24 hours refrigerated

IV bags are stable 8 hours at room temp, 16 hours refrigerated

Bolus/IV

Slowly inject 10 ml SWFI (100 mg vial) onto inside wall of vial.

Gently swirl each vial (about 3 minutes). Discard if not in solution.

Transfer into empty polyolefin or polyvinyl chloride IV bag (250 ml or less)
